The Hospital Safety Grade measures hospitals on how safe they are for patients.
The Leapfrog Group creates the grade using data from Centers for Medicare & Medicaid Services (CMS), the Leapfrog Hospital Survey, and the American Hospital Association’s annual survey. Taken together, they produce a single grade representing a hospital’s overall performance in keeping patients safe from preventable harm and medical errors. The Hospital Safety Grade includes 30 measures, all currently in use in national measurement and reporting programs.
